Player Health Sheets

Player Health Sheets are to be filled out by parents and returned to the team trainer. Player Health Sheets are to be available at every practice and game; held by the team trainer or head coach (in the absence of the trainer), and are to be destroyed at the end of each hockey season.

Hockey Canada Injury Report (HCIR)

Injury reporting, follow-up and proper injury analysis and treatment are important components of being a trainer for BLOMHA. An Injury Report form must be completed for each case where an injury is sustained by a player, spectator or any other person at a sanctioned BLOMHA activity. All injury reports are to be submitted to BLOMHA office staff and the Director of Risk Management.

Injuries resulting in a player/person being taken to a hospital are to be reported to BLOMHA within 24 hours of the event. Once a player has recovered from injury, they are required to have a Return to Play form filled in by a physician stating they may return to play, and must they have participated in at least one (1) practice prior to playing in a game.

Concussion Awareness

Concussions are a serious injury and should be treated as such. Being aware of the signs and symptoms of a concussion and following the proper return to play procedures is very important. Rowan's Law : Concussion Safety provides the most up to date information, including signs and symptoms, and protocol available within minor hockey.
