2024-2025 BLOMHA Referee Application
Please fill out and submit this application to Referee BLOMHA hockey for the 2023-2024 season. If this is your first season or you are new to BLOMHA, we will notify you of the next steps in late August or September.

Applicant Information

A referee must have at minimum, Level 1 HCOP Certification, and if you are over 18, a valid Police Clearance with Vulnerable Sector Screening. If you do not have a current Police Clearance on file with OHF, then the required documents must be submitted to OHF in order to complete a certification or Re-certification. Returning officials MUST register for a recertification clinic and complete the required training before accepting game assignments. A Police Clearance is valid for three seasons. If you have a Police Clearance dated 2021 or older, it is invalid. Police checks must be e-mailed to OHF [email protected] for verification before you can complete certification process. Referees must be at minimum fourteen (14) years of age to officiate in BLOMHA. Upon acceptance, new referees will be required to successfully complete a minimum Level 1 HCOP certification.

Certification, Experience + Availability

PLEASE READ CAREFULLY:

I have read and understand the BLOMHA Referee policies and the Alliance Referee Policies and Code of Conduct. I agree to adhere to these policies and accept that disciplinary action may be imposed for failing to follow policies. Disciplinary action may include fines, suspension and/or dismissal.

I acknowledge that as an official for BLOMHA, I am not an “employee” of BLOMHA but I provide my services on a freelance basis. 

Injury insurance is provided by Hockey Canada, not BLOMHA. I understand that HC injury insurance is supplemental insurance for costs that exceed what is covered by OHIP and my personal insurance policies. Any and all claims are administered by Hockey Canada

I agree that BLOMHA, its Board of Directors, Officiating Committee, the Referee-in-Chief(s) and/or Referee Assigner will not be financially responsible for injuries incurred while officiating in games assigned by BLOMHA.

I am aware that regular meetings take place that communicate special rules, announcements, procedures and training for officials. I agree to attend these meetings on a regular basis.

I understand that a condition of receiving assignments is my availability and I am required to make my services available for a reasonable amount of time each month when BLOMHA games are taking place.

I understand that I may be required to officiate games early Saturday/Sunday mornings (as early as 7:00 am) on a regular basis and my refusal to do such games may result in disciplinary action up to and including dismissal.

I understand that games assigned to me during times that I declared as “available” and/or games accepted by me are my responsibility. I accept that excessive cancellations of my assignments may result in disciplinary action up to and including dismissal.

Applications for the 2024-2025 season will be accepted after June 1, 2024.  

Submission of an application does NOT guarantee an officiating position with the Burlington BLOMHA Officials. Acceptance is also dependent on successful completion of annual Hockey Canada Re-certification.

Applications for returning referees must be received no later than August 31, 2023. Returning referees will be notified of acceptance in September

Applications for new Officials to BLOMHA will be accepted until August 31, 2024.
