ALLIANCE Hockey Vaccination Policy

To our Bulldog family: Alliance Hockey has passed policies mandating all eligible participants to be double vaccinated in order to participate in BLOMHA hockey programming.


Alliance Policy Statement: (Page 6) ALLIANCE Hockey will require proof of COVID-19 immunization for all vaccine eligible participants (12 years of age and above and unless exempted) including players, team officials, game officials, staff and volunteers in order to participate in ALLIANCE Hockey sanctioned programming. Eligible participants will require proof of having received their first dose by September 17, 2021 and be fully vaccinated by October 31, 2021.

All participants must be fully vaccinated for COVID-19 by October 31, 2021, which must include the two-week period required to be considered a complete series (second dose prior to October 17, 2021).

The timelines to complete this task are tight so please read how we need you to proceed.

Our responsibility under this policy is to collect proof of vaccination and communicate eligibility for participation with our coaches. We will not share medical information and coaches will be instructed not inform anyone else why a player is absent. We will also communicate to the parents of players that have not yet submitted the correct documentation. In relation to the government of Ontario’s announcement, BLOMHA will allow participants until 11:59 pm, September 27 to submit their documentation.

For those choosing not to participate due to this policy please fill out BLOMHA's Refund Request. Per our refund policy there will be an administration fee applied to all refunds.

Your role is to submit your proof of vaccination via one of the following on-line forms (files are stored on a protected and encrypted server and will be destroyed once no longer required).


Submitting your documentation:

1. Retrieve your proof of vaccination. In compliance with Alliance policy the following forms are considered acceptable evidence of vaccination:

    a) A digital or physical Dose Administration Receipt
    b) Provincial Government issued verification passport

2. Submit proof using our on-line forms.
