    Player Medical Info Form
    All trainers are required to have the entire team fill out this form, return it, and keep them on hand for all team events. These forms will be sent along with the player if they need to receive medical attention and their guardian(s) is not immediately available.

    REP & SELECT Preliminary Injury Form
    Please fill out this form immediately - it can be done on your phone at the rink, but it is imperative that it's completed ASAP. It is sent to the Dir. of Risk Management who will connect with the player's family to check in and discuss if further steps are necessary. Please ensure you also fill out a Hockey Canada Injury Report form within 30 days where necessary.

    2025 Injury Report
    Please note that this form is two sides and both sides MUST be filled out. If you do not fill out both sides, the player will not receive any insurance benefits that they may be entitled to. This injury report must be brought to the BLOMHA office within 30 days of the injury occurring or you can email it to [email protected] (U8-U13) or [email protected] (U14-U18).

    Return to Play form
    This form MUST be filled out any time a player sustains an injury that relates to their head. This includes activities outside of hockey as well. If a player is complaining of head pain whatsoever, this form is REQUIRED.

    Hockey Can Rule 2.1
    Please read the following bulletin from the Ontario Hockey Federation to better understand the rule around the number of Certified trainers needed for a game to go forward. This is not a suggestion but mandatory minimum for a game to be played. Trainer's must be properly certified as per Alliance guidelines. Please see the document "Trainer Certifications 2025-2026" for certification information and course links.
