Player Medical Info Form
All trainers are required to have the entire team fill out this form, return it, and keep them on hand for all team events. These forms will be sent along with the player if they need to receive medical attention and their guardian(s) is not immediately available.

Hockey Canada Injury Report form
This form is to be used for all house league injuries and any injuries that require documentation at the rep and select level. If you are submitting this form from a rep or select team, ensure you have submitted the BLOMHA Preliminary Injury Report form.
